Grant Application

Please apply below for a charitable giving grant. Proposals are accepted on a rolling basis and reviewed quarterly. Applicants are encouraged to include annual reports, brochures and/or other materials that will assist in evaluating the organization and its proposal. 

Applicants requesting AED battery and/or ECG pad replacement should specify brand and model/serial number of AED currently utilized.  Priority is given to those applicants that have not received AED placement from the Foundation in the past.

Funding decisions are based on available resources and the degree to which proposals meet Iowa Heart Foundation charitable giving priorities and review criteria. Organizations may submit only one application each year. Applicants can expect to be informed of funding decisions within three months of submitting their proposal.

  • Include facility mission; age, population, and numbers served per month; type(s) of business or activity organization supports, etc.

  • Include organization’s mission; age, population and numbers served per month; type(s) of business or activity organization supports; describe cooperative relationship between project partners.

  • Describe how the grant award will be utilized in the proposed project.

  • Upon submission of this form, you will be contacted by an Iowa Heart Center representative and asked to provide a copy of the organization’s latest 501(c)(3) IRS determination letter or a copy of a W-9 form (dated within past 12 months) to determine financial need (net income/expenses for project).
